Jeffrey R. Ginther, MD 2003 Billing Policies

It is customary to pay for charges at the time of service. Cash, checks and credit cards are accepted. While we will bill health care insurance companies directly and assist in the claims process, remember that payment for services is the patient’s responsibility and the patient is responsible for any balances not paid by insurers. We recommend that patient's verify their coverage with their insurance carriers prior to receiving medical care.

Patients with Insurance:

1. Dr. Ginther accepts most health insurance plans. We will bill health care insurance companies directly and assist in handling claims. Patient co-pays are expected at the time of service. Deductibles must be paid in a timely manner.

2. We do not bill third party insurers (e.g. auto insurance). Patients must arrange for payment.

Self-pay Patients:

Dr.Ginther accepts discounted fees from insurance carriers and is willing to negotiate a discount with those without insurance or are self-insured.

1. Office Visits: Patients are eligible for a 25% discount if payment is made at the time of service; unless treatment is an emergency, patient must reschedule appointment if not prepared to pay. (Minimum payment on day of service: $75)

2. Fracture Care: Fracture care fees include all follow-up visits for 3 months. Also included in this fee is any casting on the first visit. X-rays and subsequent castings are additional charges. Patients are eligible for a 50% discount from established fee if _ of fee is paid in advance and account is paid in a timely manner. Discount will be taken upon final payment. Accounts that are turned over to collections will be charged the full fee. (Minimum payment on first day of service: $150)

3. Surgeries: Patients are eligible for a 50% discount from established fee if _ of fee is paid in advance and account is paid in a timely manner. Discount will be taken upon final payment. Accounts that are turned over to a collection agency will be charged the full fee. (Collection Agency charges Dr. Ginther 33% of collections).

4. Extenuating Circumstances: If you have extenuating circumstances and are unable to pay a bill, please discuss these with Dr. Ginther and establish a payment plan. If payment plan is not adhered to, account will be turned over to collection agency at the full fee.

Returned Checks: A $30.00 fee is charged for all returned checks.

Collection Agency Fees: All accounts turned over to a collection agency will incur an additional $20 fee and 8% interest will be applied to all unpaid balances.

No Show Fee: A fee of $25.00 is charged for “no-shows” on previously scheduled appointment. These time slots could be rescheduled for other patients.

$15.00 Fee Minimum (additional fee for lengthy forms)
$15.00 for the first 10 pages (.25 per page over 15)

Please allow 5-7 business days to complete insurance forms.
